If you're a patient, and you or someone your care for needs help adjusting to the emotional, social or personal changes associated with an illness, our highly-trained medical social workers may be able to help.
Baptist Health Home Care's medical social workers may assist you in:
- Arranging for community resources to assist you in your home, including home-delivered meals and transportation
- Crisis intervention and counseling
- Assistance with long-term care planning, and counseling and assistance with long-term care placement, if needed
