Request Patient Records (for Affiliate Providers)
For Healthcare providers and HIPAA-covered entities who need access to patient records for treatment and continuity-of-care purposes.

To request medical records for continuity of care purposes, please use our online request form:
Outside providers may also fax (859.624.2339) a written request on letterhead with patient identifiers, their return fax/secure email, specific records/date range, and a note that the purpose is treatment/continuity of care.
After submission, if all required information is complete, the request will be processed within one hour.
