Dr. Evans earned a medical degree from Wake Forest University School of Medicine. He completed a residency in urology at San Antonio Uniformed Services Health Education Consortium.
Dr. Evans is board-certified in urology. He is a member of the American Urological Association, the Society of Government Service Urologists and the Kentucky Urological Association.
