Baptist Health MyHealth Mobile App

Purpose

The purpose of this Privacy Policy lets you know what information we collect about you when you use our application, how that information is used, and the ways in which we use the additional information you provide.

Baptist Health attempts to minimize the amount of your personal or health information stored or retained on your device. Nevertheless, our applications may collect the personal information that you may volunteer while using our services.

• Device information, such as the device used to access or use the online services, including the hardware model, operating system and version, unique device identifiers, and mobile network information. If your device settings permit, we may also collect information about the location of your device, system and application software, and personal information.
• In addition, in order to provide you certain features, our application may request information from servers and systems owned or operated by Baptist Health or its third-party vendors, and those servers and systems may record technical information about that request such as an IP address. Our application will also request one or more times your permission to turn on Location-Based Services, enabling you to better locate care facilities and doctors near you and more easily arrange transportation to a care facility, should you choose to access this service.

Baptist Health Location Services employs the use of your background location. Location Services must be enabled on your device in order for this technology to be employed by Baptist Health. You can enable or disable these services on your device at any time. Baptist Health utilizes Location Services in order to provide the following experiences while at any of our My Baptist Health Facilities that support the technology.

• Check-In Now service informs our greeting staff that you are approaching or on your way to our facilities
• Appointment arrival utilizes your phone's location to notify our staff when you've arrived

Your Location Services information is anonymous to us, is not stored, nor is it shared or sold in any way or fashion. It is only utilized by Baptist Health with your permission by enabling the services on your device and while at our facilities to render the listed services.

When using the Baptist Health MyHealth app, certain data may be temporarily cached to support functionality.

MyChart within Baptist Health MyHealth

All personal health information associated with this mobile app is securely stored on encrypted servers managed by Baptist Health or its authorized third-party vendors. These systems comply with applicable healthcare data protection regulations.

If you connect your MyChart to Google Fit, Health Connect (by Google), or Apple Health, your health and fitness data is securely transmitted and may be saved in your medical record. We do not store any health and fitness data within Baptist Health MyHealth. If connected, the MyHealth app may collect and display health metrics. These metrics may include, but are not limited to:

• Respiratory Rate
• Basal Metabolic Rate
• Body Fat
• Height
• Lean Body Mass
• Weight
• Basal Body Temperature
• Hydration
• Nutrition
• Vo2 Max
• Blood Glucose
• Blood Pressure
• Body Temperature
• Heart Rate
• Heart Rate Variability (RMSSD)
• Oxygen Saturation
• Resting Heart Rate

The data within MyChart is only accessed by authorized personnel involved in your care and is not used for marketing, or shared with third parties without your explicit consent, unless required or permitted by applicable law.
